FYSA ID CARDS


All Park Volunteers Must obtain a FYSA ID card, to participate in the leagues,
held at the Fishhawk Sports Complex and
IDs Must be worn at All Times, when on the fields, sidelines,
batting cages and dugouts.
Those in violation will be asked to leave the fields.

To obtain a FYSA ID Card, please follow the directions below.

Coaches:
includes all team managers, coaches, assistant coaches, instructors or
anyone who will be on the fields, instructing players, during games and/or practices.

1.) Submit a background clearance application, provided by your league.

2.) Obtain your Coaching Certification. You can obtain a coaching certification
by attending one of the Free Hillsborough County classes*(see the county schedule below)
or take the online course** at your own expense. (see below)

Note: At this time, the Hillsborough County Class and they NAYS online course,
are the only County approved classes FYSA will accept.


3.) To obtain a Fishhawk Youth Sports Association (FYSA) ID card.

-Your name must be on an approved background clearance, provided by your league.
-You will need to bring your county or NYSCA coaching card, or a receipt from NYSCA, showing your membership number, course completion, for verification.
-You will need to pay a $5 processing fee. (Cash or Check, made payable to FYSA only)

Volunteers/Non-Instructors
includes anyone who will be in the dugouts or on the fields supervising players, or maintaining fields.

1.) Submit a background clearance application, provided by your league.
2.) To obtain your FYSA card, your name must be on an approved background clearance list,
provided by your league.
3.) Pay a $5 processing fee, to FYSA (cash or check, made payable to FYSA only)

ID's processed in the baseball board room, next to the restrooms.

Monday April 9   5:30-7:00pm
Thursday April 12 5:30-7:00pm

Hillsborough County Coaching Certification Class Schedule, for 2012
***You MUST Pre-Register for attending this class**

An Online Course is available, at your expense, of $25.

You will need to join the web site, pay the $25 fee, choose Baseball Instruction. After you complete the course, be sure to print a copy of your certification, including the member ID number. Please make copies, to provide to the FYSA ID processor, to obtain your FYSA ID Card and to provide to the Coaching Coordinator, upon applying for a manager/coach
